NHC Advantage is an HMO I-SNP with a Medicare contract. Enrollment in NHC Advantage plans depend on contract renewal.

Out-of-network/non-contracted providers are under no obligation to treat NHC Advantage members, except in emergency situations. Please call our customer service number or see your Evidence of Coverage for more information, including the cost- sharing that applies to out-of-network services.

Some of the benefits mentioned are part of a special supplemental program for the chronically ill who are diagnosed with cancer, diabetes, cardiovascular disease, autoimmune disorder, dementia, and many other conditions. Not all members may qualify to use these benefits.

Medicare beneficiaries may also enroll in NHC Advantage through the CMS Medicare Online Enrollment Center located at http://www.medicare.gov/

Every year, Medicare evaluates plans based on a 5- star rating system.
